Living Will or Advance Directive

Lewistown Hospital will accept and comply with any valid advance directive or "living will" that describes an individual's wishes with regard to the use of life-sustaining treatment. The Hospital recognizes that an advance directive is not required to forgo or withhold life-sustaining treatment including cardiopulmonary resuscitation.

Advance Health Care Directive may contain both: Health Care Power of Attorney and a Living Will. Your advance directive should reflect your wishes. Lawyers are not required and a notary is not required in Pennsylvania.

A POLST  is an effective way to communicate the wishes of seriously ill patients to have or to limit medical treatment as they move from one care setting to another.  A POLST form in both English and Spanish is provided below.
