Geisinger–Lewistown Hospital will accept and comply with any valid advance directive or "living will" that describes an individual's wishes with regard to the use of life-sustaining treatment. The Hospital recognizes that an advance directive is not required to forgo or withhold life-sustaining treatment including cardiopulmonary resuscitation.
Advance Health Care Directive may contain both: Health Care Power of Attorney and a Living Will. Your advance directive should reflect your wishes. Lawyers are not required and a notary is not required in Pennsylvania.

Pennsylvania Orders for Life-Sustaining Treatment (POLST)
A POLST is an effective way to communicate the wishes of seriously ill patients to have or to limit medical treatment as they move from one care setting to another. A POLST form in both English and Spanish is provided below.
