Health, retirement, paid time, and other valuable benefits

This benefit package is a significant contribution over and above basic compensation.  Below is an overview of the benefits available to employees of Lewistown Healthcare Foundation.  Lewistown Healthcare Foundation includes:  Lewistown Hospital, Family Health Associates of Lewistown Hospital, Health Enterprises, Inc., and Lewistown Ambulatory Care.

EMPLOYEE SELECT PLAN (ESP) INSURANCE OPTIONS 

Insurance option premium costs are shared by the Foundation member Employers and the Employee.  Costs can vary based upon the option selected and the employee's status.

Coverage under the ESP plan is effective the first of the month coinciding with or following the employment date unless noted otherwise.
The choices and options available to employees include:

HEALTH PLAN COVERAGES

Highmark Blue Shield PPO-200:
The Highmark Blue Cross (PPO-200) is a Preferred  Provider Organization with varying coverage levels depending on the provider, and includes hospitalization and surgical procedures, diagnostic testing, emergency care, and office visits.

  In network -
$20.00 copay for office and therapy visits; $50.00 copay for Emergency Department visits; most other services are covered in full after a $200 deductible per individual (2 per family) is met.

  Out of network
- $400 deductible per person (2 per family); most services are covered at 80% of "Usual Customary Reasonable Charge" (UCR) after the deductible(s) is met.  The employee pays 20% coinsurance (in most cases) until the out-of-pocket maximum of $3000 per person (2 per family) is met, then payment is 100% of allowable charges.

Highmark Blue Shield PPO-500:
The Highmark Blue Shield (PPO-500) is a Preferred Provider Organization with varying coverage levels depending on the provider, and includes hospitalization and surgical procedures, diagnostic testing, emergency care, and office visits.

  In network - $20.00 copay for office and therapy visits; $75.00 copay for Emergency Department visits; most other services are covered in full after a $500 deductible per individual (3 per family) is met.

  Out of network - $1000 deductible per person (3 per family); most services are covered at 70% of "Usual Customary Reasonable Charge" (UCR) after the deductible(s) is met.  The employee pays 30% coinsurance (in most cases) until the out-of-pocket maximum of $3,000 per person (3 per family) is met, then payment is 100% of allowable charges.

DENTAL, VISION, AND PRESCRIPTION DRUG PLANS

Basic Dental - Provides payment of a percentage of the UCR for covered expenses.  Diagnostic and preventive care are paid at 80%.  Many other services are covered at 50% to 70% of UCR.  The maximum individual benefit per year is $1,200.  Lifetime orthodontic benefits of $1,200 per covered eligible individual are available after six months.

Enhanced Dental - The percentage of coverage is higher in some categories under the Enhanced Dental Plan compared to the Basic Dental Plan.  Diagnostic, Preventive and Basic Restorative services are covered at 100%.  This upgrade to Enhanced Dental is included in some options of the ESP program and can be elected on an employee paid basis in others.

Vision - Covered employees and their dependents are entitled to one eye exam each benefit period.  The standard benefit period is 24 months for adults 19 and older, and 12 months for children under 19.  Adults 19 and older can choose a new pair of glasses or contacts every 24 months.  Children under 19 can choose new lenses every 12 months.  Frames are covered once every 24 months for any age.  This benefit is included in some of the options under the ESP Plan, or may be purchased separately under other options.

Prescription Drug Plan - Enrollment in a Health Plan includes prescription coverage through the Total Life Care (TLC) Pharmacies.  There is a formulary that applies with three categories of covered prescriptions, each with an applicable copay.  The three levels with co-payments are:  Generic Drugs/$10.00 co-payment; Preferred Branded Drugs/$25.00 co-payment; and, Non-Preferred Branded Drugs/$35.00 co-payment.  One co-payment normally provides a 30-day supply of the prescription.
 

LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D) INSURANCE

Full-time employees are eligible for these benefits at no cost after six months of continuous employment.  Part-time employees may purchase benefits after six months of continuous employment
The amount of the life insurance coverage for a full-time employee is equal to the employee's prior year W-2 earnings rounded to the nearest $500 (to a maximum of $150,000).  The Accidental Death and Dismemberment Insurance coverage is equal to the Life Insurance amount.
Full-time employees are eligible for twice as much coverage paid for by the Foundation in some options; they may also purchase the additional amount at group rates.  Part-time employees are also eligible to purchase coverage of $10,000 at the group rates.

HEALTH CARE REIMBURSEMENT ACCOUNT (HCRA)

This option allows an employee to elect an account that may reimburse the employee for any health-related, eligible expenses that are not covered under an insurance plan.  This account is funded by an employee's pre-tax contributions and can result in tax savings for the employee.

DEPENDENT CARE REIMBURSEMENT ACCOUNT (DCRA)

An employee may elect this account in order to be reimbursed for expenses incurred for dependent care services that allow the employee (and spouse) to work outside the home.  The account is funded with the employee's pre-tax contributions and can result in tax savings to the employee.

PAID TIME BENEFITS

NON-BARGAINING AND UNITED STEEL WORKERS BARGAINING EMPLOYEES:

PAID TIME OFF (PTO) AND CATASTROPHIC ACCOUNT (CAT):  The Foundation provides a Paid Time Off (PTO) benefit for eligible non-bargaining and United Steel Workers bargaining full time and part time employees to allow a flexible bank of hours for vacation, holidays, personal needs, and short illness.  Accrual of these benefits begins upon employment.  Full time employees receive 23 days for less than 6 years; 28 days after 6 years; and 33 days for 12 or more years of service.  Exempt employees have immediate access to PTO; non-exempt employees have access after successful completion of a 3-month probationary period.  RNs, Medical Technologists, Respiratory Therapists receive 28 days upon hire, then 33 days after 6 years.  Part time employees receive prorated PTO hours based on their hours paid.  Cash payout of up to 8 days per year in lieu of taking time off is available.

The Catastrophic Account (CAT) component of the program for non-bargaining and United Steel Workers bargaining employees provides a paid time benefit for longer periods of illnesses.  Employees are eligible to access the CAT benefit after successful completion of a probationary period of three months.  Maximum accrual is 60 days.  Accrual for full time non-bargaining employees is 9 days per year.  Part time employees accrue on a prorated basis.

SEIU HEALTHCARE PENNSYLVANIA BARGAINING EMPLOYEES: 

Vacation - SEIU Employees receive two weeks per year, three weeks after six years, and 4 weeks after 12 years for full-time employees; part-time eimployees are on a pro-rated basis.  Bargaining full and part-time employees may use vacation after six months of continuous employment.

Holidays - For SEIU employees there are nine paid Holidays per year for full-time employees; part-time employees have pro-rated Holidays.  Full time and part-time employees are eligible for holidays upon successful completion of the probationary period.

Personal Days - SEIU employees receive one personal day each calendar year.  Eligibility for this benefit occurs after successful completion of the probationary period.

Sick Leave - SEIU full-time employees accrue twelve (12) paid sick days per year up to a maximum accrual of 60 days.  Accrued days are pro-rated for part-time employees.  Sick leave may be used after successful completion of the probationary period.

ELIGIBLE NON-BARGAINING AND BARGAINING EMPLOYEES:

Bereavement - Up to four days from the date of death to the day after the funeral is allowed for specific family members.  Both full-time and part-time employees are eligible for this benefit.

Jury/Witness Duty - A full-time or part-time employee is eligible to receive pay for the period of jury/witness duty which causes the employee to lose regularly scheduled work.

U.S. Military Reserve/National Guard - An employee is reimbursed the difference between the military pay for a two-week period and the employee's regular pay for scheduled hours during the same two-week period, when the military pay is lower.

RETIREMENT PLANS

Lewistown Hospital Defined Contribution Plan - (can also be called a 403b or Tax Sheltered Annuity (TSA) Plan) provides for employer and voluntary employee contributions. Lewistown Hospital employees may make voluntary employee contributions. The plan’s Employer contributions only apply to Non-Bargaining and United Steelworkers’ bargaining unit employees. Voluntary contributions can be made immediately upon hire and are fully vested. To be eligible for the employer paid contribution you must be age 21, work 1000 hours in a plan year (January 1 through December 31), and be employed on the last day of the plan year. Employer contributions are fully vested after three plan years of participation.

Employees covered by the Service Employees International Union (SEIU) bargaining unit are covered by that union’s defined benefit pension plan. The Hospital contributes to the plan on behalf of the employees. SEIU employees may make voluntary contributions through the Lewistown Hospital Defined Contribution Plan, but because they are covered by the union plan, are ineligible for the employer component of that plan.

Family Health Associates 401(k) Plan - this defined contribution plan provides an employer paid benefit to employees of Family Health Associates. It also allows for voluntary employee contributions with a 50% employer paid match on the first 6% of salary. Participation begins the pay following the date of hire.

Health Enterprises, Inc. 401(k) Plan - Health Enterprises, Inc. employees are immediately eligible to participate in this program that is employee paid through payroll deductions. There also is a employer paid contribution for qualifying employees. To qualify for the employer payment, an employee must be age 21 and be employed at least 500 hrs during a plan year (January 1. through December 31). Employee contributions are immediately vested and employer paid contributions are fully vested after three years. Contributions are tax-sheltered until retirement.

Social Security - The Foundation matches the employee’s contributions to the Social Security program.

LONG TERM DISABILITY INSURANCE

The Foundation pays the entire cost of the Long-Term Disability Plan for full-time employees.  Coverage is effective after six months of continuous employment.  The Plan provides for 50% of gross earnings after a three-month period of disability.  The minimum benefit is $50 per month and the maximum is $4,000 per month.  This benefit complements the Foundation's Sick Leave and Catastrophic Account Programs.

OTHER PAID BENEFITS

EMPLOYEE ASSISTANCE PROGRAM

An employee becomes eligible for the Employee Assistance Program after successful completion of the employee's probationary period.  This program provides up to six hours of counseling for the employee and for each of the employee's eligible dependents every fiscal year.  This program is paid for by the Foundation.

EDUCATIONAL ASSISTANCE

After successful completion of an employee's probationary period, the Foundation reimburses 75% of a full-time employee's tuition for approved job related courses.  For part-time employees the reimbursement is 37.5%.  Employees must receive a grade of "C" or higher.  The maximum annual reimbursement is $1000.

UNIFORM ALLOWANCE

An annual uniform allowance is provided to employees who are required to wear uniforms by Lewistown Hospital.  This benefit is paid for by the Hospital and is available to full-time and part-time employees after one year of service.
